End-of-Life Care

Palliative Care

Dignified, compassionate support for people with life-limiting illness — delivered at home, where you are most comfortable, with your family close.

Overview

Comfort, dignity, and support at end of life

Palliative care is a planned, holistic approach to supporting people with life-limiting illness. Central Home Care provides specialist palliative care at home, focusing on comfort, dignity, and quality of life — for both the person receiving care and their family. You will never go through it alone.

Our Approach

Five key areas of our palliative care

1

Advance Care Planning

We work with you and your family to document your care preferences and wishes, ensuring your voice is heard and respected throughout your care journey — even when you can no longer communicate them yourself.

2

Communication & Family Support

We keep everyone closely informed and involved. Our team records family contact details and ensures clear, compassionate communication at every stage — whether you are a partner, child, or close friend.

3

Holistic Needs Assessment

We assess and address spiritual, pastoral, emotional, cultural, and symptom management needs — recognising that end-of-life care is about the whole person, not just physical symptoms.

4

Comfort & Symptom Management

Our team manages pain, breathlessness, nausea, and other physical symptoms to keep your loved one as comfortable as possible. We also record and honour personal comfort preferences.

5

Equipment & Practical Support

We ensure the right equipment is in place at home — from hospital beds and pressure mattresses to personal care items — so care can be delivered safely and comfortably.

6

Grief & Bereavement Support

Our support extends beyond the person receiving care. We provide access to counselling and grief support for families, both during the care journey and after.
